All participants are required to have a completed Image and Creative Work Release Form on file. Participants who are under 18 years of age must have this form completed by a parent or legal guardian.

Please indicate whether you agree to or opt out of having your child's image and work used by New York University.

I agree to having my child's image and work used by New York University.

I opt out of having my child's image and creative work used by NYU, and I will provide my child's program with a current photo so it can be used to identify my child for removal from recorded media.

Please note the following:

By agreeing, I hereby give my permission to New York University (NYU) to use photographs, videos, and audio/visual recordings in which my child's picture, image, or likeness appears, and to use any creative efforts including stories, artwork,performances, and projects my child may produce through their program for all purposes and in all media including, without limitation, brochures, advertisements, and other promotional materials.

Please note the following:

By opting out, I do not wish New York University (NYU) to use photographs, videos, and audio/visual recordings in which my child's picture, image, or likeness appears, nor to use any creative efforts including stories, artwork, performances, and projects my child may produce through their program for any purpose and in any media, including without limitation, brochures, advertisements, and other promotional materials. I understand NYU will make reasonable efforts to comply with this request. I also agree to provide my child's program with a current photo so that it may be compared to any recorded media taken for the purpose of excluding my child's likeness. Finally, I will notify NYU if I become aware of a recording with my child's likeness, and I understand NYU will then make reasonable efforts to remove my child's likeness from the recording.
