College & Career Lab: Exploratory Stage - Track 2 Returner Enrollment Form

Enrollment Instructions

To maintain your enrolled student status for College & Career Lab, CCL students must re-enroll for the Exploratory Stage Track 2 Summer 2024 session. Students will be prompted via email to complete the Program Forms (Image Release, Risk and Liability, and Contract) and ensure their Treatment of a Minor form with the NYU Student Health Center is up to date for the summer. 

Students may need the assistance of an adult to accurately report emergency contact information. You cannot save your work on this form and return to it later. You must complete and submit the enrollment form in one setting. It should take no longer than 5 minutes to complete.

NOTE: This enrollment form is for returning CCL Exploratory students who participated with CCL last summer. If you have not participated with CCL before, please fill out the new student application form instead.
